5.7 ACCIDENT/INCIDENT REPORT

Whenever a patron or staff member is injured on Library property or when any staff witnesses or participates in an unusual incident on Library property, an accident/incident report should be completed and submitted to the library’s Regional Office.


The report is to be completed as soon as possible following the accident/incident and sent to the Branch Supervisor. After review it needs to be submitted to the Library’s Regional Office for the Director to review.


It is important to include, whenever possible, the names and telephone numbers of all persons involved or witnesses for possible follow up action.


Examples of accident/incidents which require reports may include injuries to patrons or staff, property damage, disappearance of equipment, disgruntled patrons, and other unusual behavior by persons on Library property.
